An iPad application intended for Rheumatology practices that collects patient reported outcome (PRO) and (optionally) clinician-derived data to collect, calculate, and store multiple quantitative, validated disease activity measure for patients with rheumatoid arthritis (RA) and other rheumatic diseases.

App Features:
• Intended for in-office collection of data from patients, and optionally from clinicians
• Collects multiple validated disease activity measures for RA including patient global, pain, fatigue, patient acceptable symptom state, rheumatoid arthritis disease activity index, rapid assessment of patient index data, clinical disease activity index (CDAI) and the disease activity score (DAS28-ESR and DAS28-CRP)
• Scores automatically, no missing information
• Data saved longitudinally and trends over time provided
• Captures drug information

Notes:
• App requires an active Internet connection, either through a data plan or a Wi-fi network.
• Basic version captures only PRO data without storing any information. Enhanced features requires custom logon available from the developer.
• Requires iOS 5.1 or newer
